The accuracy of the information provided above and requests the opening of a client account with Collection Design BV.
They have read, understood, and accepted the General Terms and Conditions of Sale (attached), the internet practices, and the proprietary reservation clauses communicated by Collection Design BV and commits through this declaration to establish a continuous business relationship with Collection Design BV under which these terms, practices, and clauses will apply, unless an agreement to the contrary is expressly agreed upon and signed by Collection Design BV and the Client.

Collection Design BV reserves the right to verify the information provided by the Client, including transmitting this information to various credit agencies to obtain a credit verification for the Client. Collection Design BV informs that the opening of an account, payment methods, and granted credit facilities are subject to verification and confirmation based on the client's credit rating in due time.

By signing this direct debit mandate form, we authorize Collection Design B.V. to send instructions to our bank to debit our account and our bank to debit our account in accordance with the instructions from Collection Design B.V.

This mandate is dedicated to SEPA direct debits. The debtor can request a refund from their bank under the following conditions:

Up to 8 weeks from the date of debit for an authorized debit.
Up to 13 months in the case of an unauthorized debit.
